Dialysis Provider Training
Explore Transplant’s Dialysis Provider Trainings have increased dialysis providers' knowledge, skils and comfort educating their own patients about transplant across the country. This training, with the educational materials provided, can assist dialysis centers in meeting the Centers for Medicare & Medicaid Services (CMS) ESRD Conditions for Coverage pertaining to transplant education.
Explore Transplant Dialysis Provider Training Objectives:
- Enhance dialysis providers’ transplant knowledge & experience delivering transplant education to patients.
- Assist providers in designing individualized transplant education plans that support the CMS ESRD Conditions for Coverage regarding transplant.
- Provide dialysis centeres with Explore Transplant education materials for patients.
- Strengthen relationships between transplant and dialysis centers to facilitate transplantation.
Also introduced is the Explore Transplant Tracking Tool for educators that includes a list of questions to ask, records key information to tailor education by patient, records patients’ stage of readiness about transplant, and shows evidence that transplant education has occurred in the event of a state or federal audit.
