Foundations of Explore Transplant

The foundation of the Explore Transplant educational program is a theoretical model of health behavior change called the Transtheoretical Model of Change (TTM). The TTM has been used successfully to promote a variety of healthy behaviors including stopping smoking, losing weight, and now pursuing transplant.

Based on this theory, patients could be at one of five stages of readiness to pursue transplant. These stages could range from a patient having no knowledge of or interest in pursuing transplant (Precontemplation) all the way to a patient already being listed for a deceased donor transplant or having a living donor evaluated for transplant (Maintenance).

Since each patient has different concerns about transplant and awareness about transplant’s advantages, the education and the delivery of the Explore Transplant education is varied to accommodate each patient’s Stage of Change.
